Should a logo (say, six inches wide by three inches tall) be just above the breast, across the breast, or even higher? Are there any specific standards for t-shirt printing?

Its really up to you where you would like to put the logo.
Stanards is left chest, full front and full back, however you can place it higher lower whereever it would look best.
I actually sell logo branded t-shirts to customers and I do the graphics free of charge.

You should place it just above the breast possibly overlapping the breast area a little bit.
