did you lost your pink copy (E-6) or your id card? you can go to the nearest SSS office and ask about it. i think they have their records stored in their system and it is online, so i think they can easily retrieve it when you report a lost id. they will ask your full name, birthday and address, the assistant will type it on her/his computer and it will appear. check their website: http://www.sss.gov.ph/
